Community Transition Facilitator

SUPERVISIONThe Facilitator works as a member of a team but reports to the Director of Regional TransitionRESPONSIBILITIESo Analyze job tasks and work environments so as to plan appropriate training, supervision and support services for clients placed in assessment or work development activitieso Collect referral information and authorizations and create case file.
Request client records and necessary paperwork to proceed with services.
o Coordinate community assessment placements with school, family, and business partners.
o Provide training and instruction that will enable clients to learn job tasks and integrate naturally into the business work routine.
o Observe and record client performance and work behaviors and complete required assessment reports.
o Liaison with business personnel regarding:
support needs of clients placed; client's work performance; and building natural supports.
o Develop and maintain relationships with business partners.
o Maintain ongoing communication with Vocational Services Coordinator regarding client evaluation or training process and progress.
o Maintain ongoing communication with ORS Counselor, school personnel and family regarding progress of client.
o Attend Intake, IEP/IPE, Assessment Review meetings as requested.
o Work with other Project staff in meeting the overall goals of the Project.
o Submit all required reports in a timely manner.
o Other tasks as assigned.
MINIMUM QUALIFICATIONSMinimum Associates degree in human services, Bachelors degree preferred.
Experience with adolescents; knowledge of developmental, mental health, physical, and learning disabilities.
Excellent communication skills and writing skills.
Must be available to work school and non-school hours and have a safe driving record and be willing to transport students and clients in agency vehicle.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
